it was a fragmentation grenade. The explosion itself isn’t strong enough to actually do that kind of damage to the human body.

Rather, the explosion fragments the metal casing of the grenade into shrapnel. Those flying pieces of sharp metal can easily shred body armor and body parts, without damaging the building.

His mouth looks like that because he caught a piece of shrapnel in the cheek. And yes, it is quite disturbing. I hope I never see the real thing.
